Request for Disclosure, etc.

NAVITIME JAPAN Co., Ltd. ("Navitime") sets forth, as follows, the procedures to deal with a request from a customer or his/her proxy regarding the customer's "retained personal data"* to:
- notify the purpose of use of such data;
- disclose such data;
- correct, add to or delete data; or
- stop the use of, erase or stop the provision to third parties of such data (collectively the "Request for Disclosure, etc.").

* "Retained personal data" means the retained personal data set forth in the Act on the Protection of Personal Information (Act No. 57 of 2003).

  1. Department to Receive Requests for Disclosure, etc.

    When a customer or his/her proxy makes a Request for Disclosure, etc., please send your request in a prescribed request form together with necessary documents to the following department by mail. Please note in advance that Navitime will not accept Requests for Disclosure, etc. made in any manner other than by mail, as strict confirmation of identification is required.

  2. How to Make Requests for Disclosure, etc.

    To make a Request for Disclosure, etc., please:

    (1)
    download the relevant form from among Navitime's prescribed request forms and fill in all of the necessary information; and
    (2)
    prepare the documents for confirming identification; and send said documents to the department set forth in the immediately preceding paragraph (Department to Receive Requests for Disclosure, etc.) by any manner where delivery is recorded, such as simplified registered mail (kanni kakitome).

    Navitime's Prescribed Request Forms

    - Request for Notification of Purpose of Use of Retained Personal Data
    - Request for Disclosure of Retained Personal Data
    - Request for Correction of, Addition to or Deletion of Retained Personal Data
    - Request for Stopping Use, Erasure or Stopping Provision to Third Parties of Retained Personal Data
    * "Request for Notification of Purpose of Use of Retained Personal Data" and "Request for Disclosure of Retained Personal Data" are subject to a fee.
    ("Request for Correction of, Addition to or Deletion of Retained Personal Data" and "Request for Stopping Use, Erasure or Stopping Provision to Third Parties of Retained Personal Data" are not subject to a fee.)
    (2)

    Documents for Confirming Identification

    Any one of the following:
    - copy of driver's license*;
    - copy of basic resident registration card with a photo (shashin-tsuki-jyumin-kihon-daicyo-kado)*;
    - copy of insurance card of health insurance*;
    - copy of passport; or
    - copy of alien registration card (gaikokujin-toroku-sho)*.

    * Only original copy issued in Japan are accepted.

  3. Request for Disclosure, etc. by Proxy

    When a proxy makes a Request for Disclosure, etc., please enclose the documents listed below in addition to the documents set forth in (1) and (2) of the immediately preceding paragraph (How to Make Request for Disclosure, etc.).

    (1)

    Documents for Confirming Identification of Proxy

    Any one of the following regarding the proxy:
    - copy of driver's license*;
    - copy of basic resident registration card with a photo (shashin-tsuki-jyumin-kihon-daicyo-kado)*;
    - copy of insurance card of health insurance*;
    - copy of passport; or
    - copy of alien registration card (gaikokujin-toroku-sho)*.

    * Only original copy issued in Japan are accepted.

    (2)

    Documents for Confirming Agency Power

    In the case of the proxy appointed by the relevant customer:

    1. one power of attorney (signed by the relevant customer)
    2. one relevant customer's signature certification
  4. Fee

    "Request for Notification of Purpose of Use of Retained Personal Data" and "Request for Disclosure of Retained Personal Data" are subject to the following fee:

    1,000 yen (including tax) per request

    Please enclose postal stamps valued at 1,000 yen with each request form.

    * Navitime may charge an additional fee depending on the substance of the Request for
    Disclosure, etc. In such case, Navitime will give notice in advance.

  5. Other Notes

    (1)

    With respect to the Request for Disclosure, etc. by a customer or his/her proxy concerning the "retained personal data," Navitime will send a reply in writing to the address described in the request form set forth in Paragraph 2, Item (1) (Navitime's Prescribed Request Forms).

    (2)

    In any of the following cases, Navitime may not provide a reply to the Request for Disclosure, etc. regarding the retained personal data. If Navitime determines not to provide a reply, it will give notice to such effect with the reason therefor (the fee will not be refunded).
    - the Request for Disclosure, etc. is made with respect to any data that is not the retained personal data;
    - the description in the request form is not sufficient to identify the retained personal data in question;
    - the request form or other documents are incomplete;
    - the prescribed fee is not enclosed or the fee enclosed is not sufficient; or
    - no reply is required under applicable laws and ordinances.

    (3)

    Any documents sent to Navitime will not be returned.
